SUMMERY OF BENEFITS: This document gives your plan’s overview including all your main benefits, such as telling you your doctor co-payments, hospital co-payments, lab costs, dental benefits and so much more.
EVIDENCE OF COVERAGE (EOS): This document gives you detailed explanations, telling you just about everything there is to know about your plan and how it works. TIP TO SEARCH IN EOS: If you click the Control Key on your keyboard and the letter F (CNRL + F) it will bring up a search screen where you can put in a word such as “kidney” and it will show you everywhere in the EOS where kidney is mentioned.
DOCTOR SEARCH: Find if your doctor is in network or search to find available doctors. You can search by doctor name or by specialty such as looking for a urologist.
DRUG SEARCH: Find out if a drug is in formulary and what tier it is. Use it to search new drugs that are prescribed to you to find out your costs. TIP: Once you know the tier for a drug look up the tiers co-payment cost in the Summary of Benefits.
PHARMACY SEARCH: Lets you check for pharmacies and see which are preferred. Preferred pharmacies will cost you far less in co-payments than non-preferred pharmacies will cost you.

REIMBURSEMENT FORMS: There are a number of things that you may be entitled to a reimbursement from Aetna for, including using an out-of-network dentist and getting some of your payment reimbursed, or getting glasses from a non-EyeMed optometrist. Use this link to access forms.

ANNUAL NOTICE OF CHANGE (ANOC) LETTER: In September. We know, you get plenty of mail from Aetna and everyone else involved with Medicare, probably way too much mail. One piece of mail, though, is really important — your ANOC letter. It will tell you exactly how your plan is going to change for the following calendar year. Changes could be minor, or they could be significant and really important. Make sure that you look out for and read your ANOC letter.
ANNUAL ENROLLMENT PERIOD: October 15 to December 7th each year. This is the period of time when you can change your Medicare Advantage plan with your new plan beginning January 1. It is a time to renew your needs including whether your plan still includes your doctors and covers your prescriptions well. We are more than happy to review your needs for the coming year with you and go over plan options for the coming year.
MEDICARE ADVANTAGE OPEN ENROLLMENT PERIOD: January 1 to March 31 each year. We often call it the “Oh No Enrollment Period.” As in “Oh No,” you thought your doctor was in your plan for the new year but he isn’t. You get one more chance to change your plan up through March 31, but if you use that change, there are no more, “Oh No’s,” it’s final. Unless . . .
SPECIAL ENROLLMENT PERIODS (SUCH AS A MOVE): These are periods of time when you can change your plan because you have a special circumstance. The most common reason to change plans is because of a move outside the plan's service area. Even if you just move to a different county, chances are you will need to change your plan. You have 60 days prior to your move and 60 days after your move to make the plan switch.
